I request `Lucky 17 Fun Casinos` to provide fun casino entertainment

I agree to pay ‘Mr. M Dalley’ a fixed fee of £for the services of `Lucky 17 Fun Casinos` between the hours of:

(Please note we will need at least 1 weeks notice of any time changes, the finish time cannot be changed on the day if, meals, arrangements, etc run over the planned timetable. Croupiers will deal for up to 3 hours which includes a 15 minute break, if additional time is required, there will be an extra charge of £45 per Hour/Table or £25 per half hour/per table)
Irrespective of the amount my guests utilize the casino entertainment.

I have sent (via paypal) a booking fee of £ which is equal to £50 per table (plus any supplements due*),
I understand that my booking fee is non refundable and that the balance of £ will be paid (via paypal)14 days before the date that the entertainment is to be provided. If the balance is not received by this date the booking will be deemed as cancelled and loss of deposit will occur.

2. No cash prizes will be awarded.
3. Chips utilized at the event have no cash value and therefore cannot be redeemed for cash. .
4. I agree to pay the cost of any damage (within 28 days of the event) to equipment or casino tables caused by any person attending my event.
5. `Lucky 17 Fun Casinos` COMPLY WITH THE LOTTERIES & AMUSEMENTS ACT 1976 UNDER SECTION 15 & THE NATIONAL LOTTERIES ACT 1993. GAMES ARE PLAYED FOR AMUSEMENT ONLY, ALTHOUGH PRIZES MAY BE GIVEN.
6. If for reasons beyond our control the casino can not go ahead a full refund will be given and no other claims will be entered into.
7. Casino nights are lawful if held as part of an “exempt entertainment”, as laid down in section 15 of the Lotteries and Amusements Act 1976. A copy of the Act can be obtained from the Office of Public Sector Information (formerly HMSO). Under Section 15, the promoters of an “exempt entertainment” (i.e.: a fete, dinner, dance, sporting event or bazaar etc) may lawfully provide as part of the entertainment any “amusement with prizes”, whether it is a lottery or gaming or both, provided the following conditions are met: That the whole proceeds of the entertainment, after deducting the expenses of the entertainment, shall be devoted to purposes other than private gain; That the facilities for winning prizes at such amusements are not the only or substantial inducement to persons to attend the entertainment.
8. Due to insurance restrictions children who wish to take part must be accompanied by a parent.
